Add the meal start deadline to the break rule

California requires the first meal to BEGIN before the end of the fifth hour. A 30-minute meal taken at hour seven is a full violation even though it was the right length, so the deadline needs its own column — a length check cannot stand in for it, and BreakEvaluator already tests MEAL_LATE against it.
