Mirrors the AccountHistory entity in kamo-shared-library. Written explicitly rather than left to ddl-auto because the defaults matter: ID and DATE_CREATED carry database defaults so a plain SQL insert works without naming them, which Hibernate's generated DDL does not do. No foreign keys, on purpose — the log outlives what it describes. An account can be merged away and a member can leave the org without the delete failing or the record of what they did disappearing with them.
