The loader effect listed departmentsState in its deps and set it to 'loading' on entry, so React tore that effect instance down immediately, the cleanup flipped cancelled, and the reply landed on a dead closure. The guard also read the same state, so nothing ever started a second attempt — the tab sat on 'Loading departments…' for good. Track whether a load has been started in a ref and keep it out of the deps, so setting state no longer cancels the request that set it.
