Drop the constraint that limited a member to one billing account

Finds the constraint by shape rather than by name — Hibernate generated it and the name is not stable across environments, so any unique constraint on account_members whose column list is exactly member_id is the one to drop. The composite on (account_uid, member_id) has two columns and never matches.
