Keep the profile heading on the current membership type

The heading reads "<Member> Profile" or "<Team Member> Profile" off the type MyAccountTab reports through onMemberTypeLoaded, which only fired from the mount-time member fetch. Promoting or demoting therefore left the title on the old label until the page was refreshed. Report the new type from the membership change handler as well.
