Appends MICROSOFT_TEAMS to VoipProviderType. The enum persists as ORDINAL on **************** so the position is load-bearing and the constant goes last; the class Javadoc now says so rather than leaving the next person to infer it. No DDL change — an ORDINAL column carries no value whitelist, so this needs no KamoInitializer run. Adds two derived queries on MemberVoipOAuthTokenRepository: • findByProviderAndInstanceId — maps an inbound provider identity (a Microsoft Entra object id on a Teams call record) back to a Kamo member. • deleteByInstanceId — these rows have no FK to the instance, so deleting a provider was leaving them behind still reporting "connected".
