Migrate subscription JSON fields to entity-based @ManyToMany relationships

Replace jsonb string columns (compatiblePlanIds, includedInPlanIds, applicablePlanIds, applicableBillingCycles) with proper JPA @ManyToMany join tables and @ElementCollection. Update service layer to resolve plan entities instead of serializing JSON, and update promotion DTOs from String to List<String> for consistency with add-on/bundle DTOs.
