Move Upload button into DocManager toolbar

The Upload trigger was only in the /doc/manager page's actions prop, making it invisible when DocManager is embedded in other pages. Moved the Upload button directly into DocManager's toolbar so it is always present regardless of context. Removed the now-redundant external button from page.tsx and the global event listener it relied on.
