Provisioning had nothing underneath it, so two simultaneous first loads both inserted. Collapses what that already made — twenty duplicate rows across seven organisations and thirteen memberships, oldest wins, losers deactivated rather than deleted and anything pointing at one re-pointed first — then adds the partial unique index per calendar kind that makes a second row impossible. Partial because the invariant is on the PROVISIONED calendars: a plain unique index on the organisation would forbid a member's own, and one on the membership would forbid the extra calendars a member is free to create.
