Additive nullable columns + org-unique transaction-number index for the MLOS originator/transaction refactor: mlos_loan_transaction (transaction_number + assigned_lo_*), loan_originators (frozen_at/frozen_state WORM markers), mlos_member_document (loan_transaction_id deal-binding). Idempotent.
