The image/video/PDF thumbnail steps set thumb flags on a detached ImgDat and called save(), which merges and cascades into the orphanRemoval images collection — throwing 'a collection with cascade=all-delete-orphan was no longer referenced' and leaving the upload stuck. Switch all three to **************** (bulk column update, no entity graph).
