§164.524(c)(2)(ii) is the form and format the individual requested if it is readily producible. Both were already producible; only one was offered. A C-CDA is what a receiving practice's system ingests. A FHIR bundle is what a patient's own health app reads, and asking for one is the common case now rather than the exotic one. The helper line says who each format is FOR instead of naming the standard and leaving the front desk to work it out. The recipient goes on both paths. On the FHIR path it is what turns the read into a recorded disclosure — the same endpoint serves the chart screens, and those must not land in the patient's accounting as releases. The format sits above the recipient, not beside the button: it is part of what the requester asked for, not a download preference somebody discovers after typing the name.
