Rewrite HexHead placement system with deterministic layout engine

Replaces the buggy collision-avoidance system (spiralSearch, resolveCollisions, flyToHomeRow, dock/group RAF loops) with a single deterministic reflow() function. Fixes hexheads appearing behind tool windows (z:100→250), overlapping each other, and inconsistent movement. Preserves honeycomb grouping, snap/swarm, and all events.
