Simplify source selector, add dirty tracking and unsaved changes dialog

- Remove department/user sub-type selectors — templates are org-level for the organization, all departments, and all users - Remove the avatar icon to the left of the Source dropdown - Save button starts disabled, enables only when changes are detected - Switching source with unsaved changes shows a confirmation dialog with Cancel, Discard Changes, and Save & Switch options
