Conversion runs on a virtual thread against a DETACHED ImgDat. save() therefore merges an instance whose LAZY `images` collection is no longer the managed one, and Hibernate aborts with 'A collection with cascade=all-delete-orphan was no longer referenced by the owning entity instance' — every upload failed at the mark-converted step, before thumbnails were generated, so documents landed with isConverted=false and no preview. The catch block then hit the same error while trying to record the reason, which is why conversionError stayed null. Adds markConverted / markConvertedWithRendition / markConversionError alongside the markThumbAvailable this repository already had for the identical reason.
