Found in the live HOLD-path test: with the engines ready, a brief hold is judged in milliseconds and the antivirus.hold.verdict event arrived ~30 ms BEFORE the upload's transaction committed its is_missing ImgDat. The completer found nothing waiting, answered NOTHING_TO_DO and deleted the hold, so the document stayed missing for good (and its triple blocked a re-upload). The completer now waits up to 20 s for the waiting row; when nothing is waiting it deletes the hold only if a stored copy exists (a dedup hit), otherwise it keeps the hold on the scanner's disk instead of discarding the only checked bytes.
