A per-mailbox add-on is not priced per user

PER_MAILBOX fell through to the default arm, so the public pricing page advertised the two e-mail add-ons as "/user/month" against a per-mailbox price. PER_EXTENSION is handled ahead of the PBX add-ons landing.
