A one-off setup fee on a quote that presents a single price. The fee already existed on QuoteOption, where each alternative carries its own; a flat sheet had no option to hang one from, so it could not record a setup fee at all. Nullable, so the ADD COLUMN is accepted on a populated table and every existing quote keeps meaning what it meant: no column, no fee.
