Three nullable ADD COLUMN IF NOT EXISTS on mcp_tool_call_audit: policy_decision, policy_rule, disclosure_event_id (spec §5.1, D11; master §6.10). Hand-applied by CP08 Task 3; a contract test pins the file's shape.
