OrchestrationState (per-loan-stage GREEN/BLOCKED/COMPLETE) + StewardTask (exception record) + enums + DecisionSpine (@ConditionalOnProperty mlos.spine.enabled): composes the shipped DecisionGate + AiDecision WORM ledger + transactional outbox in one tx — ROUTE_TO_HUMAN opens a task + BLOCKs the stage; resolve() writes a HUMAN_OVERRODE decision + unblocks. Mutable state entities (no @PreUpdate). 4/4 Mockito tests green.
