File a patient record under a CRM account (ehr_account_patient)

The account page's Healthcare tab lists the patient records filed under an account and, through the patients' own portal proxies, the other records they may see. A link, not a column on the patient: one record can sit under two accounts (a child on both parents') and one account holds a family. Linking grants no access. Table created by hand as the owner role (2026-11-07), indexes valid.
