Fix avatar display with override mechanism support

- ProfileTab: Always show USER avatar (no override mechanism) - MembershipPermissionsTab: Always show MEMBER avatar (no override mechanism) - Site-wide displays (chatheads, chatboxes, member lists): Respect override mechanism - If member has memberAvatar â†’ show member avatar (override) - Otherwise â†’ show user avatar (default) - Remove hardcoded /cam/1.jpg fallback images - Update fetchMemberAvatar to properly respect override logic - Simplify MemberItem to use avatar URL directly from endpoint response
