The plan records what shipped; this records why it differs from the plan. Three rulings were themselves corrected by a later review (the Stripe write count went fifteen -> thirteen -> fourteen -> twenty, and the keying rule went 'every write' -> 'creates only' -> 'durable creates only'). Those corrections are recorded in place rather than tidied away, because the sequence is the useful part.
