Leave unique keys to the entities instead of duplicating them

Eight migration statements created unique indexes on exactly the columns their entities already declare unique (BillingGroupMember, BillingGroup, MailboxBillingDay, ExtensionBillingDay, PersonalMailboxEntity, **************** MemberImapConfigEntity, MemberNotificationPreference). ddl-auto applies those keys before any runner, so every table carried two identical unique indexes, each its own tablet. The duplicates were dropped from production on 2026-09-16; stop recreating them. Tests now pin the keys on the entities and fail if a migration re-adds one.
