Let a coder session run at the model and effort the member picked

for a new session and for a resumed one alike, since `--model` and `--effort` are per-session flags either way. They are the ONLY part of the command a caller can influence, and they still cannot express a flag: both go through `coder_flags`, the same fixed vocabulary the Send-to-AI dispatch uses, which maps a name onto fixed arguments and raises on anything else. The permission mode is not reachable from here and does not become a parameter. Naming NEITHER is allowed and means "whatever the member's own configuration says" — a better answer than one this agent invented, and what any caller that predates this sends. Naming only ONE is refused: running at their model but somebody else's effort is not what either answer meant. Ultra Code is xhigh plus a keyword in the MESSAGE, and an interactive session has no message for the agent to put it in. It therefore runs at xhigh and the member types the word themselves, which is the honest behaviour rather than a silent downgrade dressed up as something else. 215 tests.
