A document is a document — extraction and the model are platform-level, not per lender. Adds a cross-tenant NEEDS_REVIEW queue + QcReviewService.platformReview (org/app derived from the fact, not the caller) so a central ops team can review every tenant's low-confidence extractions into ONE training corpus + ONE model. Tenant self-review (own loans) remains, refactored onto a shared applyReview.
