Put per-file upload settings inline to halve card height

The Description/Folder/Doc Type/Access Level fields sat on a second line under the file row, so every pending upload cost ~108px. Move them into the top row between the filename and the action buttons, so a card is one line. The row wraps, so the settings group drops to its own line on narrow panels instead of crushing the filename; the group also wraps internally. Locked (queued/active) cards keep their chip row unchanged.
