An assembled price is a base cell PLUS the adjustments stacked onto it, so judging only the cell judges half the number: a cleanly-read 99.875 with a misread -1.250 LLPA on top is still a wrong price. MlosPriceAdjustment now carries the read quality of that one adjustment, so the sign-off gate can take the weakest link across the whole assembly. MlosGuidelineVersion gets the same field for consistency with the rate-sheet spine -- it already carried sourceDocumentId, so this completes its provenance. Both nullable; null means NO SIGNAL rather than good. Columns applied to prod CockroachDB via KamoInitializer.
