Refactor leads view: fix overflow, replace score with comms card, add layout

- Add layout.tsx with flex constraint chain matching pipeline pattern - Fix overflow bug: use **************** hierarchy instead of 100vh which didn't account for app shell - Remove lead score card and score trend chart (and recharts dependency) - Replace insight cards with Docs, Open Tasks, History (clickable to tab) - Add Communications card with Notes/Calls/E-Mails/Texts breakdown + total - Content card now properly fills remaining viewport height
