Rename apps DNS alias to apply on the DNS setup studio

The apps.<domain> host serves the loan-application and patient-portal pages, not app downloads, so relabel the /setup/dns card and reserve "apply" instead of "apps" for new org aliases. Point the patient portal invite link and the CRM Vendors & Products Application link helper text at apply.<domain> / apply.kamocrm.com.
