Show JustCall webhook callback URL on new-phone setup

Surfaces the per-instance callback URL on the JustCall new-phone and overview tabs so members know what to give JustCall when configuring APIs and webhooks. The URL is fetched from the server, shown with a copy button after create, and explained on the form before the phone server id exists.
