A document has one modified record, so the Modified tab has one row per document. The column names the layers touched rather than which of two kinds a row was: Document once its bytes differ from the original's, Form Fields once fields have been laid over it, and "Not edited yet" for a record that was derived by opening an editor and then nothing was done. That last state is real and worth saying: opening an editor derives the record, so claiming a layer there would tell the member they edited something they only looked at.
