Strip SchemaCleanupRunner to only post-Hibernate fixups

Hibernate ddl-auto: update handles all table/column creation from JPA entities. Removed all redundant CREATE TABLE/ADD COLUMN statements that duplicated what Hibernate does automatically. Only kept: - Staging tables (no JPA entity) - Partial unique indexes (Hibernate can't generate) - Data migrations (role renames, one-time fixups) - CockroachDB nullable FK default workaround - Column/table drops for removed entities
