Let entrypoint handle install, not startup.sh

FreePBX installer (php install -n) requires Asterisk to already be running. Our startup.sh was calling it before Asterisk started, causing "Error communicating with Asterisk" crash. Now startup.sh only waits for MariaDB and patches Apache ports, then execs the image entrypoint which starts Asterisk first, then runs the installer on first boot.
