Appended at ordinal 5. **************** is EnumType.ORDINAL and carries no CHECK constraint, so appending is safe and inserting anywhere else would silently re-point existing rows at a different provider — PbxProviderBillingCoverageTest pins the order from the other side. Billed THIRD_PARTY: the organization holds its own JustCall subscription (their API needs at least a Team plan) and JustCall carries the calls, so this is an integration charge, not hosting. PbxBillingClass.of has no default arm, which is what forced the decision rather than letting a new provider bill nothing forever.
