Visitor chat attachments carry scan{}; refusals name the file

The public chat widget's send answer and history rows now carry the same scan {status, family, outcome} the member frames do (verdict as the conversation's organization sees it), so a held or locked file shows as such instead of a link that answers 423. Chat upload and link-existing refusals add a top-level fileName so the composer locks the right file.
