The catch-all exten => _X.,1 in [from-internal-custom] was intercepting every internal dial — softphones started reporting "phone is not registered" / calls would not place — because Goto(from-internal,${EXTEN},1) wraps back through includes that re-include from-internal-custom, which Asterisk treats as broken/recursive on certain dial paths. The two predial-hook contexts **************** and macro-exten-vm-predial-hook) are FreePBXs documented hook points and exit cleanly with MacroExit() — they don't alter the dial flow, only add a side-effect MixMonitor() per channel. That's what the project spec called for; the from-internal-custom block was an over-reach.
