Replace upsertSigners with full delete+insert to handle frontend temp UUIDs

The frontend assigns temporary crypto.randomUUID() values to newly-added signers before any backend call. The previous upsert-by-uid logic would throw 404 for these non-existent IDs. Full replace (delete all + insert all) is the correct semantics: the client always sends the complete desired list, and the response returns the canonical DB-assigned UUIDs.
