Practice-wide revenue cycle already lives under /commerce; the PER-PATIENT half had no screen — the front desk could not see who pays before a visit, and nothing showed whether the practice was allowed to bill at all. THE RESTRICTION BANNER LEADS THE PANEL. Under §164.522(a)(1)(vi) a patient who pays out of pocket in full can require the service is not disclosed to their health plan. Billing anyway is a disclosure they explicitly forbade — it is a right, not a billing preference, and it has to be visible before somebody submits a claim rather than discoverable afterwards. The banner also says what the restriction does NOT block: a referral, a medication list, or records reaching a clinician treating them. Reading it as "hide this from everyone" withholds a medication list from the person about to prescribe against it, turning a compliance rule into a safety problem. A coverage that has never been verified says so rather than showing a blank — it is a front-desk task, not missing data. An unpaid claim shows its timely filing deadline, which is the number that decides whether the money is recoverable at all.
