Opening TacoBell from /network put ?org=kamocrm.com on the new tab. The tab lands on /validate, which renders inside this shell BEFORE its session exists — so the server has nothing to resolve, falls back to the platform, and OrgProvider records that fallback as the tab's theme domain. OrgUrlSync then stamped it into the URL as though it were the answer, claiming the platform for a tab on its way into somebody else's organization. It now writes nothing on /validate or /logout, nothing before the tab holds a session, and nothing on a render that fell back — the layout says which those are. A placeholder in the URL is worse than none: every later request from that tab is resolved from it, so a transient wrong answer becomes a permanent one.
