Name K3's NOT_ENROLLED and a missing phone system instead of misreading them

Found by SP05 Task 36 in production (sp00verify): - ApprovalClient read every 404 from K3 as "not deployed". SP10's K3 is deployed and answers 404 {reason:"NOT_ENROLLED"} for a member without runtime state, so an AI actor was told approvals do not exist yet. Only a reason-less 404 now means NOT_DEPLOYED (as PackSupport.notDeployed, SP05-T29); a 404 with a reason answers APPROVAL_UNAVAILABLE with it. - phone_call_history answered REJECTED "Unprocessable Entity" when the organization has no phone system: VOIPService's only 422 on that path is ProviderNotConfiguredException with Spring's bare error body. It now answers REJECTED {reason:"PHONE_NOT_CONFIGURED"} in words a model can use. Ruling SP05-T36.
