Stored assistant conversations are the reads least able to afford a 14-day, unqueryable log stream: the module sits outside the compliance boundary, so prompts about borrowers are persisted indefinitely and a §164.528 accounting has to be assembled from exactly this trail six years later. recordDisclosure already fails closed on a write it cannot make, which only means something once the write lands somewhere durable. Declares the JpaPhiAccessLogWriter bean and adds com.kamo.z.shared.phi.audit to @EnableJpaRepositories in the same commit. The library leaves the writer unannotated so ~40 services do not all construct it, which makes the bean and the scan entry a single indivisible change — the bean alone leaves PhiAccessLogRepository with no bean definition and CrashLoops the service.
