Asterisk 21 removed the Macro() application family outright, and FreePBX reaches these hooks with Gosub() (extensions_additional.conf: **************** The stock hook FreePBX ships is `exten => s,1,return()`. With MacroExit() the channel died on "No application 'MacroExit'" the instant before the trunk Dial(), so EVERY outbound PSTN call failed — no INVITE ever left the box and no CDR row was written. The recording MixMonitor() had already started, which is why the failure left no trace anywhere an operator would look. Found while commissioning the Telnyx trunk on k1m1: the outbound route and trunk were both correct and the call still vanished. Live host patched and dialplan reloaded; an outbound INVITE now reaches Telnyx and gets a real answer from the carrier. The "macro-" context names stay as-is — that is FreePBX's naming, not a statement that these are macros.
