Top-level org bypass + Free baseline fallback for orgs without subs

The org-level entitlement check **************** walks subscriptions targeting an org. With the new target_org model, orgs that don't yet have a targeting AccountSubscription saw all non-core apps denied. Two fixes: - Top-level KamoCRM org bypasses billing entirely — every non-core service is entitled. KamoCRM is the platform, not a customer. - Orgs with no targeting subscription fall back to the Free plan's feature matrix on the top-level org, preserving the default-true-on-missing-row behavior so legacy sub-orgs still see their apps.
