Complete rewrite of connection lines with direct DOM manipulation

Key changes: - Use direct DOM manipulation (setAttribute) instead of React state for line updates - Single rAF loop runs continuously and updates all path elements every frame - Store path refs in Map and update d attribute directly in animation loop - Use refs for connection/item data to avoid stale closures in rAF - Remove CSS transitions (don't work for SVG d attribute anyway) - Smooth frame-rate-independent animation phase calculation - Mouse position tracked in ref, updated on mousemove, used in rAF loop - Much more efficient: no React re-renders needed for line updates - Lines now update in real-time during drag, not just on drop
