- Remove silent exception swallowing in Avatar Creator avatar creation - Add proper error handling and rollback for failed uploads - Delete database record if file upload fails to prevent orphaned records - Add success logging for upload operations - Ensure upload errors are returned to client instead of being ignored - Fixes issue where avatar records were created but files were missing in MinIO
