A custom form is a set of extra questions attached to a market, asked when somebody creates a lead in it. Solar leads need roof age; insurance leads need vehicle details; neither belongs on the standard lead record.

Getting the structure right first time
Read the concept article before you build. A form is a set of stacks, and each stack holds fields. Every field needs an identifier, which is what the answer is stored against — and changing an identifier later means the answers already collected under the old one stop lining up. Ten minutes on the structure saves an afternoon.
Attaching a form to a market
Building a form doesn't put it in front of anybody. That's a separate step, covered in How to Assign Custom Forms to a Market.
