Update all fallback feature lists and gate Pipeline/Chat/LeadStatus

Root cause: layout.tsx and org API route had hardcoded fallback feature lists with only 6 old apps (missing VOIP, AI_SUPPORT, SUPPORT, KB, NOTES, DOCS). When the backend fetch failed, these stale fallbacks were used, hiding all new app UI. Updated both fallbacks to include all 10 completed-stage apps. Also gated: Pipeline (NavPri + homepage) by isCrm, Lead Status toggle (NavTop) by isCrm, Chat Now button (NavTop) by isChat.
