Resolve member→extension before VM/CDR provider lookup

CallController was passing memberId where FreePBX (and other PBX-style providers) expect an extension number — voicemail listing returned empty because no PBX extension matches a kamo memberId. Resolve PhoneExtension by (instanceId, memberId) up front and pass extension.extensionNumber to the provider; if the member has no assigned extension, return [] instead of round-tripping the provider with garbage. FreePBXProvider.getCallHistory now filters fetchAllCdrs to rows where the caller's extension is the source or destination, infers inbound/outbound requested page.
