PER_EXTENSION already renders as "extension/month" - addOnUnitLabel handles it and needs no change. The icon map does: it is keyed on the lowercased catalog code, and an unmatched key is not an error, it silently falls back to a lightning bolt. Only the two new PBX codes are added here. Three older entries in that map (kamo_email_hosting, video_conferencing, additional_storage) do not match their seeded codes either and are left alone rather than guessed at.
